Procedure to request disclosure of
retained personal data

(1) Contact details

SoftBank Corp. and WILLCOM Okinawa Inc. (collectively referred to as “the Company”) will accept requests for notification of the purpose of use of retained personal data, disclosure, correction, (correction, addition, or deletion), suspension of use, (suspension of use or deletion), or suspension of provision to a third party of retained personal data at the following address.

Complaints regarding the handling of personal information can also be directed to the following address.

Other methods for requesting disclosure

If the person making the request for disclosure, etc., is the individual, the person making the request for disclosure should receive the request form described in 1. from the Personal Information Inquiry Desk, fill in all the prescribed items, enclose the documents described in 2. for identification, and send it by registered mail to the address designated by the Personal Information Inquiry Desk. Those who wish to make a request for correction, etc., or discontinuation of use (hereinafter referred to as “request for correction”) should enclose documents indicating the reason for the request for correction. In addition to the documents listed in 1. and 2. Please refer to the request form for the items that are subject to disclosure.

  2. Documents for identification (with the exception of student ID cards, which must show the applicant's current address).
    Copies of driver's license, passport, health insurance card, physical disability certificate, student ID card, basic resident registration card, medical care certificate, mental disability health welfare certificate, alien registration certificate, and pension certificate.

(3) Requests for Disclosure by a Representative

If the person making the request for disclosure, etc., is a legal representative of a minor or an adult ward, or a representative authorized by the person making the request for disclosure, etc. In addition to the documents mentioned in the preceding paragraph, please receive the documents mentioned in 3. below from the Personal Information Inquiry Desk, fill in all the prescribed items, and send them by registered mail to the address specified by the Personal Information Inquiry Desk, enclosing the documents mentioned in 4. Please enclose the documents listed in 4. below and send them by registered mail to the address specified by the Personal Information Inquiry Desk.

In the case of a legal representative

(4) Disclosure and notification of purpose of use Fee and method of collection

For each request, please transfer 1,676 yen (including tax) to the bank account indicated on the request for disclosure, etc. form.
If the fee is insufficient, or if we are unable to confirm that the fee has been transferred, we will notify you to that effect. However, if payment is not made within the prescribed period, we will assume that no request has been made.

(5) Method of response to requests for disclosure

  1. For requests for disclosure and notification of purpose of use
    1. A.
      Commencement of Procedures

      (ⅰ) When the person who made the request for disclosure, etc., is a subscriber to SoftBank's telecommunications service (excluding those who use only the prepaid service), please submit a “Request Form for Notification of Purpose of Use” or “Request Form for Disclosure of Personal Information” (referred to as “Request Form for Disclosure, etc.”) and documents for identification designated by SoftBank. (In case the request is made by a representative, the Company shall notify the individual that the representative has the intention to request disclosure or notification of the purpose of use and that the representative is the representative himself/herself. In addition, the Company may confirm the identity of the person by calling the contact indicated in each request for disclosure and confirming with the person in question the details prescribed by the Company. The Company will then begin the procedure.

      (ⅱ) If the person who made the request for disclosure, etc., is not a subscriber to SoftBank's telecommunications service, or if the person is a user of the prepaid service only, SoftBank will confirm that the request for disclosure, etc., has been received and that the fee has been transferred (in the case of a request by a representative, SoftBank will confirm that the representative is the person himself/herself). In addition, the Company may confirm the identity of the person by calling the contact indicated in each disclosure request form and confirming the prescribed contents to the person in question). The Company will then begin the procedure.

    2. B.
      Method of response

      The Company will respond to requests for disclosure, etc., by sending a written document to the address indicated by the person who made the request for disclosure, or his or her representative in the address column of the request for disclosure form via personal mail.

    3. C.
      Cases in which we do not respond to requests for disclosure

      In the following cases, all or part of the items to be disclosed will not be disclosed. If we decide not to disclose, we will notify you to that effect with the reason. In addition, the prescribed fee will be charged even in the case of non-disclosure.

      (ⅰ) When there is a risk of harm to the life, body, property, or other rights or interests of the individual or a third party.

      (ⅱ) There is a risk of significant hindrance to the proper conduct of our business.

      (ⅲ) When it is necessary to cooperate with a national agency or a local government in executing affairs prescribed by laws and regulations, and there is a risk that notifying the person of the purpose of use or announcing it publicly may hinder the execution of such affairs (excluding the case of disclosure of retained personal data).

      (ⅳ) When it would violate any laws or regulations other than the Act on the Protection of Personal Information (except in the case of notification of the purpose of use)

      (ⅴ) When the identity of the person cannot be confirmed.

      (ⅵ) When the representative's authority of representation cannot be confirmed.

  2. In the case of a request for correction
    1. A.
      Start of procedure Start of the procedure

      When we receive a “Request for Correction of Personal Information” or “Request for Suspension of use of personal information” (referred to as “Request for Correction”), a document for identification designated by us, and a document indicating the reason for the request for correction, (in the case of a request by a representative, we will confirm that the individual has the intention of making the request for correction, to the person himself/herself. In addition, the Company may confirm the identity of the person by calling the contact indicated in each request for correction, etc. and confirming the prescribed contents to the person in question). The Company will then begin the procedure.

    2. B.
      Method of response

      The Company will respond to the request for correction, etc., by sending a written document to the address indicated by the person who made the request for correction, or his or her representative in the address field of the request for correction, via personal mail.

    3. C.
      Cases in which the Company will not respond to a request for correction

      In the following cases, the Company will not respond to all or part of a request for correction, etc. In the event that we decide not to respond to a request for correction, etc., we will notify you to that effect with the reason.

      (ⅰ) When the identity of the person cannot be confirmed

      (ⅱ) When the representative's authority of representation cannot be confirmed

      (ⅲ) When no reason to respond to the request for correction, etc. can be recognized

(6) Purpose of use of personal information obtained in connection with requests for disclosure

The Company will use personal information obtained in connection with such inquiries, only for the purpose of responding to disclosure requests based on the Act on the Protection of Personal Information.
